Lifekind Health is looking for a full-time Behavioral Health Care Manager in our Victorville clinic to play a critical role in coordinating and managing comprehensive behavioral health services to improve patient outcomes and quality of life. Collaborate closely with healthcare providers, patients, and community resources to develop and implement individualized care plans that address mental health and substance use disorders. Oversee patient progress, facilitate communication among multidisciplinary teams, and ensure adherence to evidence-based practices and regulatory requirements. By monitoring treatment effectiveness and adjusting care strategies as needed, this role aims to reduce hospitalizations and enhance patient engagement in their recovery journey.

About the position:
- Conduct comprehensive assessments of patients’ behavioral health needs and develop personalized care plans
- Coordinate and monitor patient care activities, including referrals to specialists, therapy sessions, and community support services
- Collaborate with primary care providers, psychiatrists, social workers, and other healthcare professionals to ensure integrated care delivery
- Track patient progress through regular follow-ups, adjusting care plans based on clinical outcomes and patient feedback
- Maintain accurate and timely documentation in compliance with healthcare regulations and organizational policies
- Provide education and support to patients and their families regarding behavioral health conditions and treatment options
- Facilitate communication among care teams to promote continuity of care and address any barriers to treatment adherence
- Analyze data related to patient outcomes and service utilization to identify opportunities for quality improvement
Our Ideal Candidate will have the following qualifications:
- Bachelor’s degree in nursing, social work, psychology, public health, or a related behavioral health field required, Master’s preferred
- At least 3 years of experience in behavioral health care management or a similar clinical coordination role
- Certification in case management or behavioral health care coordination (e.g., CCM, CPHQ) preferred
- Strong knowledge of mental health disorders, substance use treatment modalities, and community resources
- Familiarity with healthcare regulations, privacy laws (such as HIPAA), and documentation standards
- Excellent communication and interpersonal skills to effectively engage with patients and multidisciplinary teams
- Proficiency with electronic health record (EHR) systems and data analytics tools
- Bilingual abilities or experience serving diverse cultural populations preferred
